Tech Tuesday – April 30: 12 Forms in axiUm and Call for Presenters at CEDR

Tip #1 – 12 Forms, 12 Forms everywhere…
As you may know 12 Forms, also known as Post Tx Eval Forms, are used to assess how well a student assesses previous treatment done on a patient. This is a reminder that 12 Forms must be approved and done with the student on the same computer where the student is logged in.
The flow of 12 Forms goes like this:
Student fills out Procedure Details and Student tabs. The Joint tab is filled out by both the student and the attending to assess the accuracy of the student’s assessment.
In this example, the student has filled out the 12 Form.
We know everything is good so far because we see the student is the User listed for that 12 Form. This means the student will get credit.
If the attending fills out the Joint tab and approves on a separate computer where the attending is logged in, the student will not get credit because the attending is the last user to manipulate the 12 Form.
In this example, the attending is signed into axiUm and manipulated the 12 Form, resulting in them getting credit for the Form, not the student. To summarize, when approving and manipulating any Form where the student needs to get credit, do so on the computer where the student is signed in.
