Richmond Health & Wellness Program featured in new LeadingAge toolkit video

The Washington, D.C.-based LeadingAge Center for Housing Plus Services has launched a new toolkit that includes a video interview with VCU School of Pharmacy assistant professor K.C. Ogbonna.
The toolkit offers guidance and resources for affordable senior housing providers and health care partners looking to improve residents’ health, safety and quality of life via coordinated services and lower costs.
In a video titled “Housing: A Valuable Venue for Training Future Providers and Managing Vulnerable Patients’ Care,” Ogbonna discusses the school’s participation in the interprofessional Richmond Health & Wellness Program with Robyn I. Stone. Stone is executive director for the LeadingAge Center for Applied Research.
RHWP partners faculty and students from the VCU Schools of Pharmacy, Medicine, Nursing and Social Work, as well as the Department of Psychology, with residents of Dominion Place, a section 8 apartment building for older adults. School of Pharmacy faculty who work with RHWP, in addition to Ogbonna, are Patty Slattum and Tish Moczygemba. Forty-eight Pharm.D. students have trained with the program, for a total of about 1,250 service hours, since its inception in 2012.
The VCU teams provide chronic disease and medication therapy management and counseling for lower-income, older adults who have limited access to care or who need coordinated care. Students have the opportunity to work with patients first-hand, in a home setting, along with health care professionals representing a variety of specialties. As Obgonna notes, RHWP puts students in real-life situations where they can learn from and model others’ expertise.
