Lyndon F. Cooper | School of Dentistry

In order to facilitate the approval and reporting of Outside Professional Activities (forms CP1 and CP2), the University has developed a web-based OPA reporting and approval system, of which the School of Dentistry is now part. The web-based approach replaces the cumbersome paper form approach that has been used here at the school for some time. Faculty will be able to easily login to the OPA web site using your VCU eID and password. The web-based system will keep an electronic record of reported outside professional activities so that the required annual reporting will be simplified. With this system, CP1s can be submitted at anytime during the year.

The CP1 and CP2 forms can now be completed online by the faculty member and submitted to the department chair, who indicates approval or denial online, automatically fowarding the forms to the dean. The dean indicates approval or denial, and the faculty member is immediately notified by e-mail.

Complete instructions for the new CP1 and CP2 process can be found on the School of Dentistry Intranet. There are also contacts listed on this page if you have a specific question about the system.
