Employees, non-Medicare retirees, and/or their enrolled spouses who participate in COVA Care (Anthem) or COVA HealthAware (Aetna) health plans can qualify for Premium Rewards that reduce the cost of premiums.  The Premium Reward amount is $17 per month for each participating employee, retiree, or enrolled spouse.

Premium Rewards for the upcoming plan year starting July 1, 2015 will be available to employees or retirees and/or their spouses who are enrolled in COVA Care or COVA HealthAware on or before July 1, 2015.

1.  If you and/or your spouse are currently receiving a Premium Reward, it will expire on June 30, 2015.  You will be required to update your online health assessment at www.myactivehealth.com/cova no earlier than March 1, 2015 and no later than June 30, 2015 to continue your reward without a break.

2.  If you or your spouse are currently enrolled in COVA Care or COVA HealthAware but not currently receiving a Premium Reward, you must (a) complete a health assessment online at www.myactivehealth.com/cova, and (b) submit a biometric screening with measurements to Active Health that includes data from no earlier than April 1, 2013.

3.  If you will be newly eligible for a Premium Reward, you must (a) complete a health assessment online at www.myactivehealth.com/cova and (b) submit a biometric screening with measurements to Active health that includes data from no earlier than April 1, 2013.

Complete the required action(s) by June 30, 2015 to begin the rewards on July 1, 2015.  If the requirements are not completed by June 30, 2015, eligible members can complete the health assessment and/or the biometric screening during the plan year. The member’s Premium Reward will become effective the first of the month after both requirements are completed, submitted, and accepted by Active Health and the state health benefits program.  VCU does not approve or disapprove premium rewards.

For additional assistance and information, please contact Active Health at 1-866-938-0349.  Eligible members without computer access may request to complete the health assessment by telephone.  Employees or spouses not yet covered by COVA Care or COVA HealthAware will not be recognized by Active Health’s online system and should contact Active Health by telephone for instructions regarding how to complete and submit health assessments and/or biometric screenings in time for rewards to become effective on July 1, 2015.
